Resumen
La inteligencia artificial generativa (IAG) está transformando la sanidad, planteando un desafío inminente: amplifica capacidades humanas y democratiza el acceso a la innovación, pero simultáneamente impone la necesidad de una mayor profesionalidad y pensamiento crítico. Su integración progresiva en la práctica clínica convive con una brecha formativa en España que se agrava al ritmo de avance tecnológico. En este contexto, la formación de los profesionales sanitarios resulta insuficiente actualmente para afrontar riesgos emergentes y aprovechar el potencial de la IAG de una manera segura y ética. Este artículo presenta un marco operativo basado en seis competencias digitales clave: desde la alfabetización básica hasta la resolución de problemas asistida por IAG, incluyendo su uso seguro y ético, la ingeniería de instrucciones (prompt engineering), la integración clínica y la gestión del dato. El objetivo no es convertir profesionales sanitarios en expertos técnicos, sino dotarlos de criterio, habilidades operativas y responsabilidad profesional para que la IAG se integre como aliada estratégica bajo estándares de transparencia y prudencia. En definitiva, el desarrollo de competencias en IAG es un imperativo ineludible para que los profesionales sanitarios lideren la transformación digital, garantizando avances reales en salud sin dejar atrás a los pacientes en este momento decisivo de cambio.

Key Digital Competencies in Generative Artificial Intelligence: Gaining Literacy Today to Lead Healthcare Tomorrow
Abstract
Generative artificial intelligence (GAI) is transforming healthcare, presenting an imminent challenge: it amplifies human capabilities and democratizes access to innovation, while simultaneously demanding increased professionalism and critical thinking. Its progressive integration into clinical practice coexists with a growing educational gap in Spain, exacerbated by the rapid pace of technological advancement. In this context, the current training of healthcare professionals is insufficient to address emerging risks and to harness the potential of GAI in a safe and ethical manner. This article presents an operational framework based on six key digital competencies: from basic literacy to problem-solving assisted by GAI, including its secure and ethical use, prompt engineering, clinical integration, and data management. The aim is not to turn healthcare professionals into technical experts, but to equip them with the judgment, operational skills, and professional responsibility necessary for GAI to be integrated as a strategic ally under standards of transparency and caution. Ultimately, the development of GAI competencies is an unavoidable imperative for healthcare professionals to lead the digital transformation, ensuring real health advances while leaving no patient behind in this decisive moment of change.
