Optimización farmacoterapéutica en ancianos institucionalizados

Resumen
Objetivo: El objetivo principal fue detectar y describir problemas relacionados con medicamentos en pacientes institucionalizados con polimedicación extrema y proponer intervenciones para subsanarlos. Los objetivos secundarios incluyeron evaluar la aceptación médica de dichas intervenciones y analizar la necesidad de reintroducción de medicamentos tras la deprescripción.

Métodología: Estudio observacional realizado en residentes ≥80 años con polimedicación extrema. Se realizó una revisión farmacoterapéutica utilizando los criterios Beers 2023 y STOPP 2023. Se elaboraron informes con recomendaciones, canalizados por las geriatras hacia los médicos de la residencia. Se clasificaron los PRM según tipo, subtipo y causa. Se analizó la aceptación de las recomendaciones y la reintroducción de fármacos posterior.

Resultados: Se evaluaron 116 residentes (68,1% mujeres; edad media: 89,2 años), con una media inicial de 13,8 principios activos por paciente. El 99,1% presentó al menos un PRM, registrándose 457 en total. Los fármacos más implicados fueron quetiapina, lorazepam, paracetamol, alprazolam y omeprazol. Los criterios STOPP detectaron más PRM que los Beers. La mayoría fueron por motivos de seguridad (44,5%) e indicación (44,1%). De las recomendaciones emitidas, solo se aceptaron 166 (36,3%). Sin embargo, se logró una reducción significativa en la media de principios activos, que pasó a 12,9 por paciente (p<0,001). Tras un mes, el 90,36% de las modificaciones se mantenían; el 6,02% requirió reintroducción, especialmente de neurolépticos.

Conclusiones: La polimedicación y los PRM son altamente prevalentes en mayores institucionalizados. Pese a la limitada aceptación médica, las intervenciones farmacéuticas mostraron efectividad para optimizar la prescripción, destacando la necesidad de fortalecer la colaboración interdisciplinar.

Optimization of Treatment in Institutionalized Older Adults

Abstract
Objective: Primary objective: to detect and describe medication-related problems in institutionalized patients with extreme polymedication, and to perform pharmaceutical interventions to resolve them. Secondary objectives: to assess physician acceptance of the proposed pharmacotherapeutic recommendations, and to examine the necessity of drug reintroduction following deprescribing.

Methods: Observational study conducted in institutionalized patients aged ≥80 years with extreme polypharmacy, residing in a nursing home affiliated with the hospital pharmacy service. A pharmacotherapeutic review was carried out using the 2023 Beers and STOPP criteria. Recommendation reports were prepared and conveyed by geriatricians to the nursing home physicians. The acceptance of recommendations and the subsequent reintroduction of medications were analyzed.

Results: A total of 116 residents were included (68.1% women; mean age: 89.2 years), with an initial average of 13.8 active pharmaceutical ingredients per patient. The most frequently involved drugs were quetiapine, lorazepam, paracetamol, alprazolam, and omeprazole. The STOPP criteria identified more drug-related problems (DRPs) than the Beers criteria, mainly due to safety concerns (44.5%) and inappropriate indications (44.1%).
Of the recommendations issued, only 166 (36.3%) were accepted. A significant reduction in the mean number of active ingredients was observed, decreasing to 12.9 per patient (p < 0.001). After one month, 90.36% of the modifications were maintained, while 6.02% required reintroduction, primarily neuroleptics.

Conclusions: Polypharmacy and drug-related problems (DRPs) are highly prevalent in institutionalised elderly people. Despite limited medical acceptance, pharmaceutical interventions proved effective in optimising prescribing, highlighting the need to strengthen interdisciplinary collaboration.
