Extended infusions of antibiotics: a review

SUMMARY

Introduction: The lack of development of new antibiotics and the alarming increase in bacterial resistance has forced the search for new therapeutic strategies with agents traditionally used, especially in critically ill patients. The extended infusion of antibiotics, is an alternative that has been widely discussed in the last decade. Studies have focused on β-lactam antibiotics and vancomycin because of its pharmacokinetic and pharmacodynamic properties.

Methods: This review contains publications obtained from databases such as PubMed, ScienceDirect and IDIS, in order to show the most relevant and actualized information needed for decision-making related to this issue.

Results: Through clinical studies has not been possible to demonstrate an absolute benefit of using extended infusions of beta-lactams and vancomycin, however, evidence shows a trend of parameters available pharmacodynamic and pharmacokinetic effective in some groups of critically ill patients nosocomial infections caused by multidrug-resistant bacteria.

Conclusion: According to the evidence of the studies and the fact that the extended infusions are at least as effective as intermittent infusions, using such strategies in critically ill patients may be beneficial. Ideally, these practices should be developed under the supervision of the Antimicrobial Stewardship Programs. Furthermore, it is necessary to carry out further research on this topic.
