Impact of the Covid-19 pandemic on the consumption of antifungal drugs and their use profile in critically ill patients in a tertiary hospital
Abstract
Introduction: During the COVID-19 pandemic, critically ill patients were at increased risk of bacterial and fungal co-infections, particularly invasive fungal infections such as candidiasis and pulmonary aspergillosis, which are associated with high mortality rates. In this context, diagnostic limitations and the severity of these infections led to the use of empirical antifungal treatment, increasing the consumption of these drugs in these patients. Therefore, the aim of the study was analyse the use of antifungal drugs before, during and after the COVID-19 pandemic and to describe the profile of their use in critically ill patients.
Material and methods: Observational, retrospective, single-centre study including all patients who received antifungal treatment during their admission to the Intensive Care Unit (ICU) in a tertiary hospital in three study periods (January-June 2019, January-June 2020, and January-June 2022). Demographic variables, variables related to antifungal treatment received and microbiological diagnostic variables were collected. To describe antifungal consumption, defined daily doses (DDD) per 100 hospital and ICU stays were calculated for the study periods.
Results: Empirical treatment was the most prescribed in the 3 periods (56.18% in 2019, 65.15% in 2020 and 56.94% in 2022). Regarding the consumption of antifungals in the ICU, in the period 2019-2020, caspofungin increased by 28.74%, isavuconazole by 98.82% and posaconazole by 1432.95%, while in the period 2020-2022, the consumption of liposomal amphotericin B increased by 77.52%, anidulafungin by 56.10% and isavuconazole by 190.08%.
Conclusions: We observed an increase in empirical prescriptions, possibly related to the published report on the increase in fungal superinfections in critically ill patients with COVID-19, even though, in our case, this did not correspond to an increase in the microbiological diagnosis of these infections. This led to an increase in the consumption of antifungals that continues thereafter, despite the fact that the number of cases of patients with COVID-19 had decreased significantly.
